Tank and Fender Identification

There is a club member that has asked me to help identify the tank and fenders on the Hodaka in the picture. Anyone?

Black Clarke Yamaha tank with a Steve Mitchell cap, TonyD front fender, Husky rear fender.

Steve was also the first guy I seen use the front half of the TonyD front fender as a rear fender like here.
