Thanks Ed, I believe you nailed it (of course). Further inspection revealed "one of these things is not like the other..." IMG_7319.jpg The lower right hole is undamaged, and that was the likely culprit - backed out enough to destabilize the cover. 100% mechanic error. That's a new failure...

Bend up the tabs on the lock washer, use an impact air gun to remove the nut (so yo don't have to hold the sprocket).
